Appointment Cancellation and Rescheduling
- We request a minimum of 24-hour notice for appointment cancellations or rescheduling. This courtesy enables us to accommodate other patients awaiting consultation.
- A fee of $75 applies for cancellations made less than 24 hours in advance or for unattended appointments. Patients are accountable for costs associated with unnotified cancellations or rescheduling.
Appointment Guidelines
- Timeliness is crucial for appointments. Arrivals exceeding 10 minutes late may necessitate rescheduling.
- Urgent situations requiring appointment adjustments should be communicated promptly to our office. While we strive to accommodate, we cannot guarantee immediate rescheduling for missed appointments.
- Frequent no-shows or missed appointments may result in discontinuing services to allocate care to other patients.
FMLA and Disability Evaluations
- Routine psychiatric evaluations for employment, disability, or legal purposes are not our focus. However, longstanding patients (minimum six months) may receive evaluations at the provider’s discretion.
- For STD/FMLA form completion, prior scheduling and payment are necessary. The fee is $150 per form, non-refundable, and does not include indirect patient care activities.
Medication Policy
- Medication management is integral to patient safety and health. We prefer addressing medication adjustments during scheduled visits.
- Prescriptions are generally issued to last until the next appointment, emphasizing the importance of follow-up visits for continuous medication supply.
- The visit frequency is collaboratively determined, considering various health and treatment factors, with a standard evaluation every 90 days for psychotropic medication continuity.
Controlled Substances Guidelines
- Our approach to prescribing controlled substances is conservative, prioritizing patient safety and adherence to regulations.
- Monthly visits are mandated for controlled substance prescriptions, with strict compliance to monitoring and screening protocols to prevent misuse.
- Patients are expected to adhere to the prescribed dosages, with any unauthorized adjustments leading to potential treatment discontinuation.
Prescriptions and Refills
- Plan for medication refills to avoid shortages, particularly for non-addictive medications. Contact your pharmacy first to check for authorized refills.
- Emergent medication needs are outside our scope; timely refill requests are crucial.
